Dr. Soto The Downey Dentist ("we," "our," or "the practice") is a HIPAA-covered entity. We protect the privacy of your protected health information (PHI) in accordance with the federal Health Insurance Portability and Accountability Act (HIPAA) and applicable California law, including the Confidentiality of Medical Information Act (CMIA).

You have the right to: access and obtain copies of your dental records; request corrections to information you believe is inaccurate; request a list of disclosures of your PHI; ask us to restrict how we use or share your information; and file a complaint if you believe your rights have been violated. Patient records are stored in HIPAA-compliant practice management software with encryption at rest and in transit. Access is restricted to authorized staff and logged. Paper records are kept in locked storage. Staff are trained in HIPAA practices annually. We do not store payment card information on our servers — card processing is handled by a PCI-compliant third-party processor.
